Bonza mate, she's a ripper 8)
Staged or serendipitous - not that it matters?

A wonderful image that I would be happy to have on my office wall were there were space. To help you understand quite what an accolade that is, I have yet to take an image of my own that I feel sufficiently proud of to display on any wall :(
Where was the shot taken?

Stellar shot that stands head and shoulders above the majority of images of this type: including my own efforts
The reflection in the rippled water is exquisite and must have required careful exposure and/or positioning to get right whilst maintaining such superb detail in the demoiselle.
Was this sheer happenstance or the result of careful planning and patience?

I like the distant composition on this, it's different from most Kingfisher shots we see.

I remember my first successful shot of a kingfisher with great fondness.

Stunning: an image that captures the ethereal beauty of the Tuscan landscape in the vicinity of Siena.

Another atmospheric shot that takes me back to my student days when I lived in Sheffield and had regular access to the Peak District and its many fine walks.

Excellent choice of viewpoint, shutter speed, DOF and stitching software to create a virtual gateway through time and space to a beckoning green place.
I'd love to see a copy hung poster size in an installation with a recording of the sounds of the water and wildlife.
Respect!

Evocative: my only criticism being that your image does not actually function as gateway to being there 8)
