If the camera-to-subject distance is changing all the time, then shutter button.
If it isn't - the subjects are fast moving but in the same place - then rear AF button. That way you don't incur the AF delay after you've focused the first time.

For me - rear AF button every time.
That allows for prefocus, manual focus and regular. Even for portraiture it allows for focus-and-reframe
Takes a little while to 'learn', but I've used it for so long I don't even think about it anymore

One thing that made me give up sooner than I might have is that I look through the VF with my left eye, which means getting the thumb on the rear AF button in between the camera and my face.

I use left eye too Chris. I think I posted something similar to your experience (probably last June) when I switched from the K-x to the K5. Doesn't bother me in the slightest now though.
The thing that does bug me though is the Liveview button being right next to the AF selector button. If I switch from central point to 5 Auto points or vice versa I end up pressing the,live view button too

Shutter button for me, I did try the rear button for a little while but it didn't offer any advantage.

I just meant that unlike in AF-S, if you press the button all the way down in AF-C it will fire whether it's in focus or not (???) whereas in AF-S unless you change the custom option, it only fires if it thinks it's in focus.

Half press shutter and AF - S. I use the quick focus ring to get about there and then fine tune.
I have though been learning this week how to set up an ACTION User mode on my still newish K5 (with continuous High shooting)to replace the lazy Sport dial setting on my old KR. I can't see there is a way to include AFC in this profile. Very little help in the handbook setting these up. I'm doing another for LANDSCAPE and MACRO out of the five available.
Hopefully then I'll have the right setting when some unexpected action requires a different approach
