Agree with Sandinista! It's a fabulous portrait, pro-level stuff, very well done! The out of focus back of the head is due to the depth of field being quite small. You can work out the DOF using online calculators like this one.
Assuming you were about 1m away from the boy, your DOF at 38mm f4.5 would only be about 10cm, so it would be pretty easy to have the back of the head out of the in-focus zone. A quick way to solve the problem would be to use a smaller aperture, say f8 or f11.

I have no problem with his ear. But his face has a bright contour on his right side. If you correct it the pic will be a really professional portrait. Well done, Maria.

The easiest way is just to change your camera to 'Av' mode on the mode dial. This is a semi-automatic mode, so you can just rotate the rear e-dial to change the aperture value. Because you're in Av mode the camera will reciprocally adjust the shutter speed to give you a good exposure. Just keep in mind that as your increase the aperture value from f4.5 to f8 (or f11 or whatever you choose), the shutter speed will go down, so watch out for camera shake.
