35MM-panorama-insert for Pentax 67

hello all. i am proud owner of a cheap used sf7. my first autofocus slr after 35 years in photography!
normally i am panoramic photography-specialist using rotating cameras. but i am looking for a special 35mm-panorama insert for pentax 67.
in josef meehans book panoramic images is an us-adress of a man constructing such an item. does anybody have this adress or informations?
this insert could be very interesting when using with polaroid-instant slide film. if we could still get it? btw: outdated ones are useless the will be deteriorated.(maybe refrigerated?)

there are informations about a certain mr koch-usa in photonet-forum but he cannot be reached. that 35mm-insert is interesting for using exotic films. i thought of polaroid polapan. but this film is hardly to get-not produced anymore. storing in refrigerator would not help since it will dry-out.
