The open window carriages of the Puffing Billy in Melbourne are specially modified. They have a couple of bars across each window.
One quite low, to stop the people from slipping trough and another a bit higher up to hold on to and also to stop one going forwards. There is also an age limit. I think it is about 6 years of age. Lower ages are not allowed to have the feet dangle out. I worked as a volunteer on the tracks of the line. One of our jobs was to clear the line of branches far enough away to stop them coming in contact with the legs of the patrons. We never in the last 40 or so years had any problems with this..
