Yes, disturbing, worrying, but carefully thought out, carefully composed and very well executed.
I would try it at a slightly smaller aperture, however. f1.8 is the cause of the distant limbs being out of focus. It could do with a greater depth of field to compliment the starkness of the lighting. (?)
