H George i,
Take a look at the pictures, not my best work because I had to take them in a mirror indoors today in a spare 5 mins I was at home and no one else was there to take the picture of set up slung across my chest..
But they do show the set up which has been faultless for almost a year and I have put the same strap arrangement on two other big lenses.
The tilting Manfrotto mount is perfect, in fact is so good if using the monopod as you can tighten it just enough to be able to swivel (tilt it up and down) while panning with the monopod , it really is a perfect answer for moving subjects .
Note: I use Pentax leather straps of my Silver K3's but any good Pentax strap will do

I have just bought the Manfrotto swivel monopod head for £17.99 from Premier Ink and Photographic; looks like it will do the job.
