Last weekend I wanted to reproduce something similar to this:

however it's impossible to get to this level of detail with a 105mm macro. What do you think the author used? Extension tubes?

I have seen shots like this before. Although not quite as harsh.
It seems to be the magnification that you are lacking.
Maybe achromats, raynox 150 / 250 ?
Smaller sensors on bridge cameras can get much better mag ratios for this type of shot.
Any idea what camera was used ?
Lighting would be troublesome too I think as you would probably end up very close to the eye.
I know what i like, If not always why.

Extensions will get you there no problem, getting the light from the side is trickier as well a patient subject.

For the lighting I guess won't be too hard using artificial lighting. About the patient yes that's totally a different issue. The guy must have built something to let the people rest their head while staying totally still. Like the machines used by the opticians. What tube extensions do you suggest me to use?
