Cokin ND8 (P121 Grad neutral Grey G2), tripod with on camera spiritlevel (so if horizontals are off, blame the bubble)
Tweaked in LightRoom to work with colours, cropped slightly to balance figure.
I use a Pentax K10D, on a MacBook with LightRoom (vers 1.3 + beta 2)
